What questions should you ask on a gemba walk?

    Short answer

The best gemba walk questions are open, specific to the work in front of you and grouped the way a tier board is: safety, quality, delivery, cost and people. Ask what gets in the way, how the operator knows the output is good, where the time went this hour and what they would change, then listen, record and follow up in the daily huddle.

    Safety and conditions

    1. Is anything unsafe here right now? Guarding, housekeeping, access, PPE, a near miss nobody has reported.
    2. Does the area match its 5S standard? Drift shows where the standard is not owned.
    3. Is the standard work for this task visible and current? If the operator cannot point to it, it is not a standard.

    Quality

    1. How do you know this output is good? The answer shows whether checks are built in or assumed.
    2. What was the last defect here, and what happened to it? Tests whether problems are contained or solved.
    3. Where does rework happen on this line? Rework is the hidden factory; people know exactly where it is.

    Delivery

    1. Are we on plan this hour, and if not, why? An hour-by-hour board makes this a fact, not an opinion.
    2. Where did the time go? Waiting for parts, information, a decision, a machine.
    3. What stopped the line last shift? Repeats point at chronic causes.

    Cost

    1. Where do you wait, search, walk or carry? The eight wastes, asked in plain language.
    2. What workaround do you use most? Every workaround is a standard that does not work.
    3. Which tool, part or document is hardest to find? Searching is the easiest waste to remove.

    People

    1. What gets in your way? Then silence. The most important question on the list.
    2. What would you change if you could? Ideas live here; the suggestion box is where they go to die.
    3. Is anyone doing a task they are not signed off for? Checks the skills matrix against the shift.

    How to use the answers

Write them down at the place, not from memory later. Each problem gets an owner and a date on the team's tier board, items the team cannot resolve are escalated to the next tier with context, and the next walk checks what changed. One question asked every day beats fifteen asked once.

    How TeamAssurance helps

TeamAssurance turns a question list into a scheduled gemba walk checklist on a phone or tablet: the leader records answers, observations and photos at the place, each problem becomes an action with an owner and due date on the team's tier board, and the pattern of answers across walks is visible in the reports leaders review.

    Frequently asked questions

How many questions should you ask on a gemba walk?

Three to five, chosen for the day's theme. The full list is a menu, not a script; a walk that works through fifteen questions becomes an interview.

What should you not ask on a gemba walk?

Anything that assigns blame or asks for a justification of a number. "Why are you behind?" closes the conversation; "where did the time go this hour?" opens it.
