Lean and daily management templates
A3, RACI, fishbone, FMEA, risk assessment and other templates with the method behind each one, in HTML you can read, print and adapt.
Safety
- Risk Assessment Template
A risk assessment template is the form a team fills in to record each hazard in a task or area, who could be harmed and how, the likelihood and consequence, the resulting risk rating, the controls chosen from the hierarchy of control, the residual risk and who owns the review. This template follows the Safe Work Australia four-step method and prints to one page.
- Toolbox Talk Template
A toolbox talk template is the one-page sheet a supervisor uses to run a short safety talk with the crew before work: the topic, three key messages, how it applies to today's job, questions to ask, the hazards or ideas the crew raises, actions with owners, and the attendance record. This template keeps the talk to ten minutes and makes the actions traceable.
- JSA and SWMS Template
A JSA (job safety analysis) or SWMS (safe work method statement) template breaks a task into steps, lists the hazards at each step, rates the risk, records the controls and who is responsible for them, and is signed by the people doing the work. Both share this structure; a SWMS is the version Australian law requires for high-risk construction work.
Risk and quality
- Non-Conformance Report Template
A non-conformance report template is the form used to record a product, process or system that does not meet a requirement and to drive it to closure: identification, a description against the specific requirement, containment, disposition of the affected items, root cause, corrective action with an owner and date, verification and sign-off. This template covers quality and safety non-conformances alike.
- FMEA Template
An FMEA template is the worksheet a team fills in to analyse how a process step could fail: the failure mode, its effect and severity, its causes and occurrence, the current controls and detection, a priority, and the actions taken. This template gives the process FMEA columns in order, the three rating scales, and a filled row so the first entry is not the hardest.
- Layered Process Audit Checklist
A layered process audit checklist is the short list of yes/no questions that leaders at every level ask at the same process step: is the control plan followed, are work instructions current, is error-proofing working, are gauges calibrated, is nonconforming product segregated. This LPA checklist gives twelve questions by area, the layer and frequency table, and how to score and follow up.
Problem solving
- A3 Report Template
An A3 report template is a single page, originally A3 paper, laid out in seven sections that walk a team through a problem: background, current condition, target condition, root cause analysis, countermeasures, implementation plan and follow-up. The left side describes the problem; the right side describes the response. This template gives the sections, the prompts and the order.
- Fishbone Diagram Template
A fishbone diagram template is a cause-and-effect chart shaped like a fish skeleton: the problem is the head, the main bones are cause categories, and the smaller bones are the specific causes a team brainstorms under each. This template uses the six manufacturing categories (machine, method, material, measurement, people, environment) with prompts, and shows how to narrow causes to the ones worth testing.
Roles and governance
- RACI Matrix Template
A RACI matrix template is a table with tasks or decisions down the left and roles across the top, where each cell holds R (responsible), A (accountable), C (consulted) or I (informed). This template gives the blank grid, the four checks to run on every row and column, and a filled example for a corrective action process.
Decision making
- Prioritisation Matrix Template
A prioritisation matrix template gives a team a repeatable way to rank options against agreed criteria. This page has two: the impact-effort grid, a two-by-two for sorting a short list quickly, and the weighted matrix, a table in which each option is scored on several weighted criteria and ranked by total. Use the first weekly and the second for the year's projects.
Standard work
- SOP Template
An SOP template is the fixed format every standard operating procedure on a site follows: a header block with number, revision and owner, then purpose, scope, responsibilities, materials and PPE, the numbered steps, the checks, the records kept and the revision history. This template gives each section in order, what to write in it, and a filled example.
