    RACI Matrix Template

    In short

A RACI matrix template is a table with tasks or decisions down the left and roles across the top, where each cell holds R (responsible), A (accountable), C (consulted) or I (informed). This template gives the blank grid, the four checks to run on every row and column, and a filled example for a corrective action process.

    Key facts

    • Rule 1: exactly one A per row. Two accountable people means nobody is.
    • Rule 2: at least one R per row, or the task has no one doing it.
    • Rule 3: use roles, not names, so the chart survives staff changes.
    • Rule 4: review it when the process changes or a task keeps falling between functions.

How do you build a RACI matrix?

    1. List the tasks or decisions, in the order they happen, down the left.
    2. List the roles across the top. Five to eight is usual; more than ten means the process should be split.
    3. Fill each cell. Blank is allowed and often right.
    4. Check each row: one A, at least one R, Cs only where their input genuinely changes the outcome.
    5. Check each column: a role that is A on every row is a bottleneck; a role with nothing but I may not need to be on the chart.
    6. Agree it with the people in the roles. A RACI nobody has seen is a wish.

    What are the common mistakes?

    • Naming people rather than roles.
    • Making the senior person A for everything.
    • Consulting everyone, so decisions wait for the slowest reply.
    • Filing it after the kickoff and never looking again.

    RACI and daily management

A RACI tells you who should own each type of action; the tier board shows whether they do. When an action type keeps being reassigned or overdue, the RACI is where the fix is made: give it a clear R and A and put it back on the board.

    Step by step

    1. List the tasks. Every step and decision in the process, in order, one per row.
    2. List the roles. Across the top, as roles not names, five to eight of them.
    3. Assign R, A, C, I. Fill the cells; leave blanks where a role has no part.
    4. Check rows and columns. One A per row, at least one R, no bottleneck columns.
    5. Agree and publish. Walk it through with the people named, then put it where the work happens.

    A worked example

A food plant kept missing CAPA closure dates. Their RACI for non-conformances showed the quality manager as A and R for every row, including implementing fixes on equipment the maintenance team controlled. Rewriting it made the supervisor R for implementation, the maintenance planner R for equipment changes and the quality manager A for verification and closure only. Overdue CAPAs fell within two months because each action on the board now had an owner who could actually do it.

    In TeamAssurance

In TeamAssurance every action, audit, project and escalation has one owner and a due date, so the R in your RACI is the person the action is assigned to and the A is the meeting leader who reviews it. Reports by owner and area show where accountability is unclear, which is the signal to revisit the matrix.

    FAQ

    Frequently asked questions

Can one person be both R and A?

Yes, on a small task. Write it as A/R. On larger tasks separate them so someone checks the work.

How many people should be Consulted?

As few as will genuinely change the outcome. Each C adds waiting time.

What is the difference between RACI and RASCI?

RASCI adds S for Supportive: people who provide resources or help the R without owning the task.

Should a RACI matrix use names or roles?

Roles. Add a separate list of who currently holds each role so the chart outlives staff changes.
